Consent to Disclose Information Form [pdf]: You must have this form in your file for your letters of recommendation to be sent out to school districts.

Letter of Recommendation Cover Page [pdf]: This form should be given to your letter writers and indicates whether the letter is non-confidential (open for your review) or confidential (not open for your review).
