Lost, Stolen or Destroyed Check
If a paycheck has been lost or stolen please notify the Payroll Department immediately. The Payroll Department will arrange a stop payment on the original check. A replacement check will be issued as soon as verification has been received from the bank that the original check has not been cashed. The employee will be required to sign a Check Replacement Request Form [pdf] stating that the original check has not been negotiated and if found it will be returned to the Payroll Department.
If a paycheck has been destroyed or is void due to the issue date, please return the check to the Payroll Department. A replacement check will be issued within 4 business days.
